On-Line Entry Forms

This site was designed to accomplish two goals: 

1. POET’S EARLY ALERT PROGRAM (PEAP) – In conjunction with the creation of an early alert program, we wanted to create a safe, secure and convenient way for members of the community to submit referral forms for students they believe to be in need of additional support. Regardless of your concerns being academic, social or personal OR if you are a faculty or staff member, we want you to let us know about the concerns you have related to a student. Please click on the “FAQ” link in the upper left hand of this page, if you would like additional information. Or, simply click on “Early Alert Program” link, just below the “FAQ,” to submit an early alert. After your submission, the form will automatically be sent to the Dean of Students office and you will receive a confirmation via e-mail (if an address was supplied).  

2. ON-LINE INCIDENT REPORT – Created for Residence Life and Campus Safety staff members, this form was designed to create a faster, secure and convenient way to input disciplinary related incidents. If you are not a member of Residence Life or Campus Safety, please do not try to access this form. After your submission, the form will automatically be sent to the Dean of Students office and you will receive a confirmation via e-mail (if an address was supplied). 

Both forms are password protected and encrypted to RC4 256 bit upon submission. You should have been given a username and password which will be requested when you select a form.
